Healthy Connections Prime Ombudsman

The Healthy Connections Prime Ombudsman promotes the rights of Healthy Connections Prime members through all phases while protecting the privacy of the member.  The Healthy Connections Prime Ombudsman Program is an independent program that helps individuals, their significant others and representatives address concerns or conflicts that may interfere with their enrollment in Healthy Connections Prime or their access to Healthy Connections Prime benefits and services. 

 

Your Healthy Connections Prime plan includes all the benefits available through traditional Medicaid and Medicare programs.  The Healthy Connections Prime plans are Absolute Total CareFirst Choice VIP Care Plus, and Molina Dual Options.  Please visit the plan's website by clicking on the plan's name.

 

An Ombudsman will work with you, your managed care plan and/or your providers to solve problems.
